A notification will be sent to UEFA media services once the form has been submitted.
Please fill in one form per individual wishing to attend the event.
Please note that only complete media accreditation requests submitted via this form will be considered. 
Make sure you specify in the next page, for which match (SF1, SF2, Final) you are requesting a media accreditation.
